Supports the professional clinical staff by providing direct patient care in the home setting and performing duties as assigned by the Case Manager. Ensures all actions taken in carrying out responsibilities support patient centered care.
Unit Specific Position Responsibilities
Performs patient care and treatments under supervision of a Case Manager.
Provides and documents nursing care including, but not limited to, activities of daily living, exercise and ambulating, specimen collection and respiratory support activities.
Transfers, lifts, and positions patients while following necessary safety precautions
Contributes to the development of the patient plan of care under direction of a Registered Nurse or Physical Therapist. Communicates information related to care of patients to other team members including changes noted.
Reinforces selected patient teaching initiated by Registered Nurse or Physical therapist.
Performs procedures as defined by department competencies.
Considers age, growth, and development when providing care.
Performs and records routine technical tasks such as vital signs, collection of specimens, and intake and output.
Assists patient while maintaining the environment in a clean and orderly manner.
Assists in duties as requested to meet the day-to-day operations of the department.
Performs other assigned duties.
Attends in-services totaling a minimum of 12 hours per year.
Assures safe, therapeutic environment for patients, families, visitors, and staff.
Participates in staff development and orientation of new staff.
Sets priorities and demonstrates organizational skills.
Reinforces selected patient teaching initiated by Registered nurse or Physical Therapist.
Performs housekeeping duties as directed and assigned.
Performs patient observation, documents patient care and patient response to care and communicates significant information to the registered nurse and other appropriate team members.
Plans, shops, and prepares nutritious meals, applying any special dietary guidelines outlined by case manager.
Performs other duties as assigned.
Qualifications, Knowledge & Experience
Required Qualifications (Including any licensure, certification, education):
Completion of 75-hour basic Certified Nursing Assistant Program or completion of at least one clinical rotation of an accredited nursing program or comparable education, training and experience and certification as a Patient Care Technician.
High School diploma or GED
Minimum of 18 years of age
Basic Life Support (BLS) training, or within 90 days of hire and must maintain throughout employment
Possession of a current valid drivers license and must maintain eligibility for coverage under Mary Greeley Medical Centers insurance policy.
Required Knowledge, Skills & Experience:
Skill in effective oral communication in English. Ability to explain procedures and give information. Ability to report abnormal conditions appropriately.
Preferred Knowledge, Skills & Experience:
Previous experience in a home care setting
Knowledge of medical terminology
