This position supports Mercy's philosophy of patient centered care by facilitating rehabilitation services to a geriatric population in hopes of maintaining or improving quality of life for each resident. Responsibilities also include providing safe and effective patient care tasks and functions under the supervision of a Licensed Nurse.
Job Specific Duties/Essential Functions
Ensures that accurate and clinically relevant information is communicated with patients, their families, and co-workers at hand offs and other times as needed. Take all measures necessary to ensure that the appropriate and involved parties have communicated necessary information in order to allow for the best possible patient outcome
Provides safe and effective patient care, including restorative care under the supervisionof the Licensed Nurse by performing tasks and support functions as delegated bydepartment.
Works with each resident as needed to maintain/increase resident coordination, strength and mobility.
Responds promptly to resident needs, call lights and emergencies as needed
Assists staff with transfers and mobility as needed.
Collaborates with therapy staff to help improve resident condition
Provides resident safety through preventative measures with implementation of bedrails, wheelchairs, transfer equipment, and other devices.
Collaborates with activity staff to lead group exercises during the allotted time.
Particpates in weekly care conferences and provides input in to care planning
Maintains accurate and real time documentation of patient care provided including activity spent with each resident
Assists in identifying quality improvement opportunities and demonstrates support for changes in practice designed to improve performance
Observes and communicates to the Licensed Nurse and changes in patient condition and pertinent information regarding the needs of the patient and/or significant others
Demonstrates effective and age appropriate communication as well as the ability to understand the needs of the patient including non-verbal communication, fears, anxieties and language/cultural barriers
Applies treatments, as designated, to residents as ordered by the physician
Organizes and prioritizes daily work, identifies need for change based on resident needs, and maintains cleanly work environment
Knowledge, Skills and Abilities
Knowledge of therapeutic and restorative exercises to maintain residents mobility
Knowledge of safety equipment
Ability to safely use adaptive equipment with residents in order to provide the highest quality of life
Ability to safely use lift equipment to transfer residents.
Works closely with elderly residents who can be confused and combative at times
Professional Experience- Required vs. Preferred
Required:Certified Nurse Assistant experience
Preferred: Restorative aide experience
Preferred: Long term care experience
Education-Required vs. Preferred
High school education required
Licensure, Certification, Registration - Required vs. Preferred
Current C.N.A. certification in active status required
Restorative Aide certificate preferred
Current BCLS certification required
