Functions as an independent private practioner in the family clinic setting with the additional responsibilities of providing crisis intervention and related services to clinic patients as directed by the physicians and physician assistants as needed. Functions as an integral member of the family practice clinic team coordinating services with Mercy Behavioral Health psychiatric staff. Accepts referrals from assigned clinic or clinics. Provides psychiatric assessment, diagnosis and therapy. Provides crisis intervention and brief therapy as indicted by the needs of the pt. Coordinates psychiatric referral. Provides ongoing support and crisis intervention to the clinic population.
Demonstrates the ability to function independently with minimal supervision in a fast paced family practice clinic environment.
Encourages staff referrals and when referral is made, follows through with phone calls and scheduling for services in a timely manner. Often meets in the medical exam room as physician determines need.
Available to clinic physicians for consultation and discussion.
Maintains own schedule, answers own phone calls and rescheduling as indicated.
Completes diagnostic assessments and progress notes in a timely manner.
Maintains professional therapy practice.
Provides mental health assessments to patients who are struggling with mental health, relationship or other health issues.
Teaches patients improved coping and stress management skills.
Helps patients and their family/support system manage the emotional impact of physical health issues.
Provides patients with referrals to community resources as needed.Obtains social history information and identifies patient needs by interviewing the patient, reviewing the medical record, conferring with physicians and medical staff and contacting other information sources as necessary and appropriate. Analysis and evaluation of patient situation
Consistently reflects advanced competency in psychosocial assessment and diagnosis.
Assesses problems appropriately in terms of scope, consequences and needed interventions.
Develops individualized psychosocial case plans based on assessment and in conjunction with other members of the health care delivery team. Implements plan and monitors patient's progress.
Coordinates/manages a broad spectrum of services. Consults with physicians, medical staff, and patient to determine patient's needs. Provides patient with information on options/resources available and strategies for utilizing these resources. Assists with all necessary referrals.
Demonstrates ability to differentiate between emergency and non-emergency tasks and responds accordingly. Provides immediate supportive counseling and necessary assistance to physicians/medical staff dealing with a crisis or emergency situation. Assists with referrals to community resources when appropriate.
Provides psychosocial counseling/treatment services to restore patients to optimum social and physical health adjustment within their capacity.
Provides factual information and materials to physicians/medical staff/patients. Possesses knowledge of public and private welfare and health agencies available to serve the community. Makes referrals promptly to appropriate agencies for services suitable to meet patient needs.
Provides clinical and administrative consultation to physicians and allied health professionals through patient evaluations, case management, in-service programs and educational presentations.
Maintains liaison with community health and social service agencies.
Prepares and maintains documentation records/reports in a timely and qualitative fashion.
Recognizes the role of a therapist in the department.
Demonstrates leadership in problem solving and program development.

Adheres to organizational and department specific safety standards and guidelines.

Familiar with ICD10 and CPT coding
Demonstrates exceptional customer service and interacts effectively with physicians, patients, visitors, staff and the broader health care community.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Licensed in the state of Iowawithin one year, as social worker, marriage and family counselor or mental health therapist.
Three years post graduate experience preferred.
Knowledgeable regarding clinical interviewing techniques.
Knowledgeable regarding principles, standards and ethics.
Basic understanding of Medicare and Medicaid regulations and child/dependent adult laws and regulations are desired.
Proof of completion of Mandatory Reporter abuse training specific to population served within three (3) months of hire.
